with predominant pustular eruptions on the limbs. Laboratory tests revealed
hypereosinophilia, hepatic cholestasis, and increased serum lipase level. A skin
biopsy revealed a subcorneal, non-spongiform, neutrophilic pustule and
perifollicular lymphocyte and eosinophil infiltration. (RegiSCAR score=6).

All three patients had positive patch tests for carbamazepine, which further
confirmed the diagnosis of DRESS syndrome.

CONCLUSION : Pustular eruptions may appear in relatively few cases of DRESS
syndrome, in particular when the causative drugis Carbamazépine. Clinicians
should be aware of this atypical presentation and should use RegiSCAR criteria
early to help make the diagnosis.
